Choosing a plan and enrolling

I am here to help with navigate the decision process with you. You may call, text, email, or click the “make an appointment” button above to schedule a time to discuss your needs and options. Prior to our meeting, please e-sign a Scope of Appointment. This is a link to my secure e-sign form.

During our call we will discuss your needs and preferences. Please have medications, list of your doctors, preferred pharmacy, and preferred hospitals handy.

Based on our discussion, I will recommend a few options. Then we can discuss pros and cons of each plan and determine which policy is best for you.

Once we have chosen a plan, I will complete the paper work and send to you for signatures.

In about a week you will have your new ID cards for your chosen plan. I will be available for ongoing support and to answer any questions that may arise in the future.

Lost your Medicare ID card or number?

  • Call 1-800-MEDICARE (1-800-633-4227).TTY users can call 1-877-486-2048. There might be a problem that needs to be corrected, like updating your mailing address.
  • Sign in to your account. If you don’t have an account yet, visit to create one. You can sign in to see your Medicare Number or print an official copy of your card.

    • In the “What do you want to do?” section, you will see an option that says Get my Medicare card. Click that link.
    • The system will ask you to re-enter your account password to proceed.
    • You will see the front and back of your Medicare card.
    • You may now download, print, or request a new card by mail.
